I don't know where to go for high quality plugs, I know Radio Shack has el cheapo ones. Check selectproducts.com, they may have them. To attach the wires to them, solder and (optional) heat shrink.

The plugs I like the best are Scosche plugs that you can get from Parts Express. You can get them gold or Platinum. These plugs have a small screw to hold the center conductor. You have to be careful when soldering that you do a quick job, otherwise you can overheat the insulation and warp it causing the center conductor to not be concentric. I haven't found a RCA plug that holds tighter than the Monster Cable plugs I use right now. Last time my car got broken into the RCA jacks from the amp stayed attached to my RCA's.

I really like Ultralink ends - they are inexpensive and well made. The EFX ones are good but I still solder them. TIP - if you are having problems, buy a female to female plug and insert the RCA before soldering. It makes it easier to hold onto and keeps the pin centred. Just let it cool before removing...
My favorite end is the Vampire Wire series with the little bat on it...
